Transcript requests may be submitted using our online transcript request form or by downloading and mailing a completed transcript request form to the CST Records Office.

There is a $20 fee per transcript. If you wish to pay using a credit card/debit card, please click the Online Transcript Request button below to access our online form. Once you’ve completed the online form, click “Submit.” From here you will be directed to enter your credit card information into PayPal to complete the online payment process.

If you prefer to pay the $20 fee by mailing a check, cash or money order, please click the Mail-In Transcript Request button below to download and print our Mail-In Transcript Request form. Completed forms and payments should be mailed to to the address below. Please make checks payable to CST Records Office.
